How to register

We have an open list and welcome requests for registration from patients living in or moving to the practice area. The N3 geographical areas covered are: Church End, Finchley Central, West Finchley and Woodside Park.

You can register by bringing along your NHS medical card or completing a GMS 1 form available from reception. It is necessary that you can provide your 10 digit NHS Number to register and the name and address of your previous GP; if you are from abroad you must also note the date of arrival in the UK.

Alternatively, you can download printable forms from the Patient Online Access Link on the surgery’s website. This also includes information on how these patients can improve their access to GP appointments and prescription requests using a PC/mobile/tablet device.

Patients now register with the practice and not a particular doctor. We will make a note on your records if you prefer to see a particular doctor or nurse.

You will need to complete a patient health questionnaire with details of your past medical history. We recommend that new patients undertake a health check with a practice nurse, especially where the questionnaire indicates e.g. management of an existing medical problem or aged over the age of 65.

Patients on long term medication will need an appointment with a GP who will check whether the medication is appropriate and add it to the medical record. It woulld be useful to bring in your medication or the latest prescription request counterfoil from your previous surgery.
